Abstract   Three patients with squamous cell carcinoma of the anal canal were treated by combined chemo- therapy and radiation therapy (FUMIR). All patients had locally invasive disease and groin node metastasis (IIIb, AJCC). The treatment protocol consisted of chemotherapy with continuous 5 day infusion of 5-FU, 750 mg/m2, and mitomycin-C, 15 mg/m2, by bolus injectin and radiation 4000 ~4500 rads in 20~25 fractions. All patients received at least two cycles of chemotherapy. Complete regression of the primary tumor and grain metastasis occured in all patients. Biopsies of the anal scar performed after complete regression revealed no evidence of residual cancer in two patients and one patient underwent abdominoperineal resection in another hospital had no tumor in the surgical specimen. One patient underwent groin node dissection and the specimen was found free of microscopic cancer. There were mild toxicities such as leukopenia, diarrhea and moist perineal desquamation which resolved by conservative treatment. Currently all patients are free of disease at 8, 15, and 28months. Thus it appears that the combined chemoradiotherapy offers a definitive nonsurgical treatment of locally advanced squamous cell carcinoma of the anal canal.
